ABSTRACT

Objective:To explore the effect of patient engagement on medication safety for patients with chronic disease through a systematic review. Methods:Relevant randomized controlled trials of patient engagement on medication safety were searched from PubMed, EMBASE, The Cochrane Library, CNKI, Wanfang and VIP database. The literature was screened according to inclusion and exclusion criteria, and data extraction and literature quality evaluation was conducted on the selected literature. Results:A total of 15 eligible studies was included. Most of the studies occurred in outpatient and home settings, and the subjects were patients with chronic diseases. Patient engagement strategies can be divided into three categories: (1) patient engagement in medical decision-making; (2) patient engagement in medication adjustment; (3) patient engagement in medication management. The outcomes of medication safety included medication adherence, medication knowledge, medication beliefs, adverse events and medication errors. Intervention strategies for patients to actively engage in medication safety significantly improved patients′ medication knowledge and beliefs, but did not improve medication adherence of patients. Conclusion:Promoting patient active engagement is an effective intervention measure to improve patients ′ perception of medication safety. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusions.

ABSTRACT

Objective To investigate the variations and clinical significance of mean platelet volume (MPV),platelet distribution width (PDW) and red cell distribution width (RDW) in type 2 diabetic patients with lower extremity atherosclerotic disease (LEAD).Methods In this study,330 patients (168 male and 162 female ones) diagnosed with type 2 diabetes mellitus were recruited.According to the ankle brachial index (ABI),all the patients were divided into three groups:patients without LEAD (Group A),patients with mild to moderate LEDA (group B),and severe LEAD (group C).And 100 healthy subjects served as control group.Blood routine, fasting plasma glucose (FPG),Hemoglobulin Alc (HbAlc),blood pressure,high-sensitivity C-reactive protein (hs-CRP),blood lipids, serum creatinine and uric acid were all measured in the patients. Results MPV,PDW,RDW in all the patients with type 2 diabetes mellitus were significantly higher than those in the control group (P < 0.05).Compared with the group without LEAD,MPV,PDW and RDW in the group with LEAD were significantly higher, with significant statistically difference (P < 0.05).Pearson correlation analysis showed that MPV,PDW,RDW were positively correlated with the index of blood glucose and lipids.Logistic regression analyses indicated that the levels of MPV,PDW,RDW,hsCRP,HbAlc and low-density lipoprotein cholesterol (LDL-C) were significantly associated with lower extremity arterial disease in type 2 diabetes mellitus. Conclusions Patients with higher MPV,PDW and RDW levels were more likely to develop LEAD than those with lower levels.MPV,PDW and RDW were important risk factors for lower extremity atherosclerotic disease in patients with type 2 diabetes mellitus.

ABSTRACT

Patients with HCV infection can develop decompensated cirrhosis, hepatocellular carcinoma (HCC), even liver failure. As a result, efficient antiviral treatment is very essential to prevent HCV-related disease progression. Newly developed direct-acting antiviral agents (DAAs) have shown safety profile, favorable tolerability, and relatively short duration, which provide an opportunity to expand the number of patients who can be treated for HCV infection. There is a need for further clinical observation and summaries for DAAs in a real world. In the era of DAAs, special patients with HCV infection still get lots of attention from doctors. This review aims at the application of DAAs in patients with HCV infection, combined with chronic kidney diseases, hepatocellular carcinoma, HBV/HCV co-infection, HIV/HCV co-infection, post liver transplantation, pregnancy, children, lymphoma and retreatment.

ABSTRACT

In this work,a highly sensitive electrochemical biosensor for the detection of trace adenosine triphosphate (ATP) was proposed. The biosensor was based on porous anodic alumina (PAA) and SiO2 nanoparticles combining with several oligonucleotides to construct sandwich structure. It was characterized by scanning electron microscopy, fluorescence microscopy, differential pulse voltammetry and electrochemical impedance spectroscopy, which conformed to the reliability of the biosensor fabrication and the feasibility of the detection. In the presence of ATP,the sandwich structures could be destroyed. The variation of the current was directly corresponding to the amount of the ATP.The application of SiO2nanoparticles could effectively reduce the background and increase the sensitivity of the biosensor. The calibration curve of ATP was obtained in the range of 0.025-0.900 nmol/L with the detection limit of 13 pmol/L (S/N=3).Also,the biosensor exhibited a good specificity.Besides, the sensor was constructed easily and possessed excellent regeneration ability. The proposed biosensor was applied in detection of real sample such as mice blood. Therefore, the proposed ATP-sensing biosensor could be expected to be applied in clinical, pharmaceutical and environmental detection.
